INVESTIGATION OF 5Y-COP AND 5Y-NPS

The Investigation Team (IT) tasked with establishing the facts, conditions, and circumstances surrounding the accidents involving 5Y-COP and 5Y-NPS leading to the determination of probable cause and contributing factors in addition to the issuance of safety recommendations as needed held its organizational meeting at Transcom House on 29 November 2016. The IT discussed the framework, the steps needed to establish a work plan as a roadmap for the course of the investigation, and the funding needs.

The framework will be in accordance with the standards and recommended practices for aircraft accident investigations as specified in the provisions of Annex 13 of the International Civil Aviation Organization, the work plan will consist of action items to conduct the factual gathering, analysis, findings, conclusions, and recommendation phases of the investigation. The estimated funding needs were established based on the needs of implementing the work plan.

Along with other matters, the IT determined that the analysis of recording devices on both aircraft would be instrumental with the development of the work plan, and steps should be undertaken for this activity to be accomplished expeditiously. The IT agreed that based on the facts ascertained to date and the timeframe for similar probes both domestically and internationally, the estimated time for completion of the investigation and issuance of a final report will be about six months.
